Participation in any Hockey Without Boundaries or OPEN RINK Pro Tour program requires acknowledgment and acceptance of the following waiver, liability, and media release terms. This policy is in place to protect the safety and rights of all parties involved and to clearly outline the responsibilities and legal boundaries of participation.
1. ASSUMPTION OF RISK & RELEASE OF LIABILITY
By enrolling in our leagues, clinics, events, or tournaments, all participants and their legal guardians acknowledge and accept the following:
Participation in physical sports such as ball and roller hockey carries inherent risks of injury, including but not limited to: bruises, sprains, concussions, broken bones, and other physical harm.
The participant voluntarily assumes all risks associated with playing, practicing, or observing hockey activities at our events or facilities.
In consideration of being allowed to participate, the participant (and/or parent/guardian) agrees to release, discharge, and hold harmless Top Cheez Sports Initiative, Top Cheez Hockey Apparel LLC, Hockey Without Boundaries, OPEN RINK Pro Tour, and all affiliated entities, staff, volunteers, sponsors, and facility operators from any liability, claims, or damages resulting from participation.
This release covers both known and unknown risks and applies regardless of fault or negligence.
2. AGREEMENT TO FOLLOW RULES & POLICIES
All participants agree to:
Follow all program rules, policies, and codes of conduct
Respect the authority of coaches, referees, and league officials
Accept any consequences resulting from failure to comply with league expectations
Parents and guardians are also responsible for ensuring that their children understand and adhere to league rules.
Failure to comply may result in warnings, suspensions, or dismissal from the program without refund.
3. EMERGENCY MEDICAL TREATMENT CONSENT
In the event of a medical emergency during participation:
The parent/guardian grants permission for the league staff or volunteers to administer basic first aid.
If necessary, the parent/guardian authorizes the program to contact emergency services and secure transportation to a medical facility.
The parent/guardian assumes full responsibility for any resulting medical expenses not covered by personal insurance.
If a parent or guardian is not present, league personnel will act in good faith to seek appropriate care and support.
4. MEDIA RELEASE
As part of our ongoing outreach and promotional efforts, Hockey Without Boundaries and the OPEN RINK Pro Tour may take photographs or video footage during games, practices, and events.
By participating in any of our programs, the participant and/or their parent or legal guardian:
Grants permission to use the participant's image, likeness, name, voice, or written statements in promotional materials
Understands that these materials may appear in printed publications, digital content, social media, videos, or other public communications
Waives any right to compensation or approval in connection with such use
If you wish to opt out of media coverage, you must notify the program in writing prior to the start of the season or event. While we will do our best to honor requests, public group settings may still result in incidental appearances.
FINAL ACKNOWLEDGMENT
By registering for any Hockey Without Boundaries or OPEN RINK Pro Tour activity, participants and their families agree to all terms outlined in this Waiver, Liability, and Media Release Policy.
