By accepting below, I certify that I am the parent or legal guardian of the participating minor, that I am over the age of 18, and that I have read and agree to the terms and conditions below.
In consideration for my child being permitted to participate in the TOP CHEEZ OPEN RINK Youth Showcase, including any related clinics, skills challenges, games, activities, media coverage, or event programming, I agree to the following:
I understand that participation in ball hockey and related athletic activities involves inherent risks, including but not limited to falls, contact with other players, contact with sticks, balls, rink surfaces, boards, equipment, weather conditions, illness, bodily injury, property damage, or other risks that may occur before, during, or after the Event. I knowingly and voluntarily assume all risks connected with my child’s participation.
I release and hold harmless TOP CHEEZ Hockey Apparel, LLC, TOP CHEEZ Sports Initiative, TOP CHEEZ Swag, their owners, officers, directors, board members, employees, volunteers, coaches, instructors, contractors, partners, sponsors, affiliates, representatives, successors, assigns, and insurers from any and all claims, losses, damages, injuries, illness, or liabilities arising from my child’s participation in the Event, to the fullest extent permitted by law.
I understand that the OPEN RINK Youth Showcase is a youth sports event and that no athletic activity is completely free from risk. I agree that my child will follow all posted rules, coach instructions, safety guidelines, sportsmanship expectations, and event protocols. I understand that failure to follow these rules may result in removal from the Event without refund.
I confirm that my child is physically able to participate in ball hockey activities. I agree not to allow my child to participate if they are sick, injured, experiencing symptoms of illness, or otherwise unable to safely take part in the Event.
I authorize TOP CHEEZ representatives, coaches, volunteers, or event staff to seek emergency medical treatment for my child if necessary. I understand that I remain responsible for any medical costs, treatment, transportation, or related expenses that may result.
I grant TOP CHEEZ Hockey Apparel, LLC, TOP CHEEZ Sports Initiative, TOP CHEEZ Swag, and their approved partners the right to photograph, record, film, or otherwise capture my child’s participation in the OPEN RINK Youth Showcase. I authorize the use of my child’s name, image, likeness, voice, photographs, video, and event highlights for promotional, social media, website, marketing, fundraising, press, broadcast, and community storytelling purposes without compensation.
I understand that the Event may be promoted through TOP CHEEZ media platforms, including but not limited to websites, social media, newsletters, flyers, video highlights, player features, and community partner promotions.
I agree to receive event-related communications, registration updates, program announcements, marketing messages, and other communications from TOP CHEEZ using the contact information I provide.
I agree that my child and I will not make, publish, or communicate defamatory, threatening, abusive, or harmful statements toward the Event, TOP CHEEZ, its staff, volunteers, participants, families, partners, sponsors, or affiliated organizations.
I understand that this agreement represents the complete understanding between myself and the Released Parties regarding my child’s participation in the OPEN RINK Youth Showcase. If any portion of this agreement is found to be unenforceable, the remaining portions shall remain in effect to the fullest extent permitted by law.
This agreement shall be governed by the laws of the State of Colorado.
